Regardless of where your program will occur, if you plan to allow young people to take the survey on a digital device (e.g., cell phone, tablet, computer) whether owned by the young person, school or your program, you must gather parental/caregiver permission to allow their children to take the survey online. The parent/caregiver cannot opt out, and must return a form to your agency. The Children's Online Privacy Protection Act (COPPA), a federal law, is intended to protect children from the potential harms of the internet.

In order to comply with COPPA, Hello Insight must also be sent a copy of these signed parental consents. Scan completed forms and email them to support@helloinsight.org. You can tailor the forms below for best use in your program.

Forms are available in Chinese (Traditional and Simplified), English, French, and Spanish.

If you are a program that operates in school or out of school, serves children under the age of 13, and are not planning to administer the survey on a digital device (e.g., cell phone, tablet, computer) COPPA is not relevant. You do not need to scan and send forms to Hello Insight. You should follow the other guidelines on this page related to distributing consent forms.
