Programs Serving Young People Ages 18 and Older

Young people 18 and older don't need parental or caregiver permission to complete surveys, but they do need to assent to be surveyed.

If you’re surveying young people 18 or older, they do not need a parental permission slip. You can either use formal permission language or you may verbally ask young when you begin administering the survey.

Forms are available in Chinese (Traditional and Simplified), English, French, and Spanish.

All young people have the right to opt out of taking a survey, not just young people 18 and older. This 'opt out' guidance in included in the Sample Script featured in our standard survey administration script.
