If you’re surveying young people 18 or older, they do not need a parental permission slip. You can either use formal permission language or you may verbally ask young when you begin administering the survey.

Forms are available in Chinese (Traditional and Simplified), English, French, and Spanish.

All young people have the right to opt out of taking a survey, not just young people 18 and older. This type of youth consent language is included in the survey administration script.
